2018 ACT Symposium—Together We Can ACT!

Join your colleagues from ACT Teams across the state! Systems are transforming with a focus on integrated care. Together we can share and learn best practices and discuss what this transformation means for ACT teams. This symposium will explore ACT as an evidence-based model; its core practices, operations and structure; ACT team dynamics, and much more.

All ACT providers in NYS, including team members, quality assurance staff, finance departments, and administrators are invited to join us.

The symposium will also provide a platform where teams can exchange ideas and learn from each other.        Workshops will help teams in strengthening their daily practice, including specialist roles. We will also have an opportunity to network while having some fun in the evening. Continuing Education Credits* will be available.

ACT Symposium Goals 

  • Participants will learn about the 8 roles of ACT Teams based on the TMACT fidelity and the Standard of Care.
  • Participants will increase skills in the areas of outreach, engagement, motivation, and transition.
  • Participants will gain deeper understanding of the 3 dimensions of ACT transitioning: engaging  consumers, managing wellness, and integration in the community.
  • Participants will gain information about trauma-informed care practices and multicultural awareness. 

To register for the ACT Symposium 2018:

  1. Please register for one or both (daylong) sessions of the Symposium.  Note that you must complete a daylong session in order to earn Continuing Education Credits* for that day. New staff members must register themselves within the Center for Practice Innovations' Learning Community in order to sign up for training. Team Leaders should forward the self-registration link and instructions they received to all staff requiring training. The link you received is unique to your team and will need to be used to register new hires for ACT training. New ACT Teams: The OUR TURN Initiative

Date: Thursday, November 1st, 2018 from 11:00AM-12:30PM (Online)

 

Description: This is a start-up training plan for new act teams. The training plan consists of the following components: 

I. ACT Required Training. The new ACT Team providers will complete the required training as they are hired on to the teams. All ACT Team providers will attend the scheduled CORE and Person-Centered Treatment facilitated by the ACT Institute. 

II. New ACT Teams Learning Community. Learning Community lead facilitator is Luis Lopez. Co-facilitation will be provided by Helle Thorning, Pascale Jean-Noel, Noah Lipton, David Lowenthal, Leslie Marino, and Sascha Altman DuBrul. 

III. ACT Role Training. Each role on the team will participate in a blended training format that consist of self-paced role learning available on the CPI/ACT LMS and a ½ day f2f training on each unique particular role. Timing of these trainings will be scheduled in September, when it is expected that all new teams have completed their hiring. After the initial start-up training, it is expected that all new providers will join the ACT Institutes role support call as they are provided for all ACT teams. As well as other training activities provided by the ACT Institute (eg. Team leader retreat, ) 

IV. Consultations. 

Who should attend? All providers on the teams, clinical directors, program managers, government?

ACT Team Leader Support Call

Pascale JeanNoel, Director of Training, hosts a Team Leader Support Call held on the first Monday of each month (exceptions will be noted).  This call time will be an open forum for Team Leaders and/or Program Assistants to ask questions or raise concerns relevant to the Team Leader role on ACT. 

Topics for discussion can include: 

  • Team training
  • Community visits
  • Treatment plans
  • Transition
  • Administrative issues
  • Accessing the LMS
